Born in Punjab, India, I migrated to the United States with my family at 9, and we settled in Eugene, Oregon, the only “brown” kid in school. I learned quickly, pouring over ESL books until late at night: The Secret Garden and The Wind in the Willows opened new worlds of captivating images and beautiful stories that I longed to be part of. Along with the new language, I adopted a unique way of dressing, unique mannerisms, and contemporary tastes, including American pop culture. Whenever taxi drivers or servers asked where I was from, I told them I was American. I asked my mom to stop packing Indian food for my lunch at home; the smell of curry I once enjoyed now embarrassed me. Instead, I wanted instant mashed potatoes and Kraft mac and cheese. Things improved after my family started traveling internationally, and I learned a lot about diverse cultures and ethnicities, becoming more comfortable in my skin. In high school, I sometimes researched my country, India, and my culture and would present in class. I learned that people appreciated the diversity and always asked me questions about India and life there. I moved to New York after getting married in 2015, basking in diversity and opportunity. Since moving to New York, my focus has gravitated towards helping the poor, vulnerable, marginalized, or disenfranchised members of my society, those who otherwise might not be able to represent or defend themselves. I am the first girl in my extended family to earn not only a master's but even a bachelor's degree. My deceased father dreamed of my earning a degree in Law. I began tutoring children while still a child myself, mostly language arts, in our temple in India and continued to do so in our temple here in America through high school, preparing curriculums for all classes, facilitating meetings, workshops, and tutoring students. I have volunteered at FOOD for Lane County (FFLC), a private, nonprofit food bank with a mission of helping to reduce hunger by engaging our community in the creation of access to food: soliciting, collecting, growing, rescuing, preparing, and packaging food for distribution through our network of social service agencies and programs. I volunteered to serve as part of a team working to raise public awareness and education and encourage long-term community-based solutions to hunger. I have also volunteered at The Eugene Mission to provide emergency assistance and long-term solutions for homeless neighbors (men, women, and children) in search of stable employment and housing. . Born and raised in India, I immigrated to America on 17 May 2008. At 29, I am a Certified Occupational Therapist Assistant and currently serve at a skilled nursing facility; highly professional and consummately dedicated to the people I do, especially our residents/patients and the staff, who have become like my family away from home. Despite the nightmare we have now been living through for weeks, if not months – it seems like an eternity – of the coronavirus targeting and killing our residents, I still could not be happier in my chosen career. I have tasted victory when an older adult walks for the first time again after having a stroke, uses a telephone without assistance, or a host of other little everyday things that we take for granted, the look of triumph in their eyes, the joy in their hearts and mine. I have grown enormously along with the coronavirus. I feel empowered by the sight on the television of 40, 50, or more elderly residents being wheeled out of their nursing homes, already lifeless, to be stacked up in line waiting for attention at the overflowing morgue. I am ready and meticulous in the care I exercise and plan for safe distancing practices over the long term. I will not abandon my work in nursing homes, and I make this pledge to myself deep inside my soul, for it is my calling, my duty, and the residents need me to help them live life to the fullest. I hope to be accepted to the MPH Program at XXXX University because earning the master's degree in public health has long been the pinnacle of my professional dreams, the area of studies in which I am confident that I will be able to make my maximum contribution to the health and well-being of my society. I also find increasing satisfaction in thinking about health issues in their global context, especially in the age of our current pandemic. While I first set my sights on becoming a nurse, my global sense of identity and urgency has led me in the direction of public health, cultivating my internationalism, traveling, and serving the underserved and unprotected in poorer nations. While I knew early on that a career in health was for me, it was not until my thinking had a chance to mature and my horizons broadened that I realized that I wanted to be a soldier in the battle against disease, but not in a clinical setting. My daydream turned away from ICU and became focused on the laboratory and work in the field to support public health initiatives in the community rather than always in the hospital. I am drawn to the broad array of options available to the healthcare worker who is qualified in Public Health, working for NGOs, corporate institutions, grass-roots organizations, and directly with vulnerable populations. My quest is to serve embedded in at-risk communities and make differences that can transcend populations across generations. As a Russian American immigrant, who moved to the United States at the age of four, I was fortunate enough to learn this valuable lesson from an early age. Hungry for success, I took advantage of every educational opportunity available. By the time I had finished high school, I had completed every advanced math course my school had offered, including Calculus, Statistics, Linear Algebra, Differential Equations, and Multivariable Calculus. I have maintained a conceptual continuity in my academic and professional career. Located at the heart of one of the most significant metropolitan cities, NYU provides an unparalleled learning atmosphere from the Real Estate industry’s leading professionals; therefore, I see NYU’s MSRE program as the most beneficial step for my continued professional advancement. My parents had always wanted me to become a doctor, and I saw the pre-dental pathway as a safe investment. While I flourished in math and science, scoring in the 99.97th percentile nationally on my DAT (Dental Admission Test), I naturally gravitated towards business, finance, and investing. I took this opportunity to enroll in the business adjunct program offered through Binghamton's School of Management. While I am devastated by the suffering caused worldwide by the COVID-19 virus, I am heartened that my being bilingual further heightens the value and urgency of what I can contribute, fomenting accurate and timely communication across demographics and health care. Seeking balance in everything I do, I always put medicine first; I love music, hiking, dancing, and family. During my clinical rotations, I remember running into classmates eager to get to the specialty rotation on which they were already focused. By the time I completed my internal medicine core rotation in my third year of medical school, I was committed to lifelong learning in this specialty, and I talked about it all the time, listening to internal medicine podcasts from the New England Journal of Medicine while driving, showering, and cooking. I invested in my favorite book, Harrison’s Principals of Internal Medicine, asking questions, and getting to know patients and their challenges. Internal medicine means that I can be everything to my patient. I can hold their hand and listen, listen to their heart, manage their blood sugar and hormonal imbalances, and if they break a leg, consult orthopedics, and continually properly educate them concerning concerns, procedures, and expectations. For the balance of my professional life, serving as a physician for many decades, I will live up to the oath to honor, preserve, and care for human life at every turn, giving all to my patients. As the entire world struggles to breathe due to the ravages of COVID-19, I feel especially called and blessed to serve in the central and critical role of those who have chosen medicine as our profession. - SLP Speech and Language Therapy Personal Purpose
I hope to become certified and practice in Speech and Language Pathology (SLP). Finding myself especially drawn to medical issues, it is my sincere hope that the healthcare community will continue to accord increasing respect and attention to the role of the Medical SLP. I consider speech therapy to be foundational to health care in every dimension since verbal communication makes every form of health care provision possible. Since I hope to practice as an SLP in the future in the Kingdom of Saudi Arabia, I focus my report on that country. Saudi Arabia has invested much of its wealth over the last few decades in projects that are seen as sustainable. Thus, the concept of ‘sustainable’ is now pervasive among Saudi professionals, including those in health care, with public health care assuming a prominent role in debates over the future of health care policy for the Kingdom. Saudi Arabia has been dramatically blessed economically with the means to provide increasingly state-of-the-art, long-term care in its medical facilities. I am pleased to see the Kingdom investing some of its resources in numerous long-term care facilities that serve as models for other parts of the world, especially the Middle East. Long-term care is the area in which I look forward to practicing as a Speech and Language practitioner, so it serves as the central focus of this report and the reflections that follow. The most advanced long-term care facilities in Saudi Arabia now provide a broad range of services and comforts for patients and residents that have not been provided in the past and are very seldom, if at all, provided in other middle eastern countries. However, some of these facilities, although considered state-of-the-art for Saudi Arabia, still do not include any role for the Med SLP. The trend, however, is towards greater recognition of the importance of that role or function, even in the long-term care facility. As I see it, Medical SLPs, serving alongside a wide range of other healthcare professionals - in addition to nurses and doctors - have a positive and critical role in long-term care. From my perspective, their position or presence is at least justified, if not necessary, for long-term care to represent our most acceptable and most sustainable effort to provide the finest possible care for the sick, aged, and disabled who experience problems communicating verbally - with professionals, caregivers, and family alike. The image above is taken from the NMC Chronic Care Specialty Hospital website in Jeddah, Saudi Arabia, one of the country's most advanced long-term care facilities, along with the caption, “Our Patient-Centric Care Model.” I see this graphic as helpful in illustrating my professional hope that the support of SLPs will increasingly become fully incorporated into strategies for long-term care in Saud Arabia tomorrow. It is exciting to understand what is missing in this graphic and what is present. Understanding what is missing in this graphic visual and what is present is exciting. In fact, what is present helps us better understand what is missing, especially from the perspective of an aspiring SLP professional who looks forward to building a career in her field in Saudi Arabia. Most importantly, I have my heart set on working in long-term care for the elderly and incapacitated. The above graphic does not yet have an image of the SLP alongside the Social Worker and the Psychologist. I hope to contribute to the efforts of a new generation of SLP professionals in Saudi Arabia so that this image will, in time, appear the full recognition of the Med SLP as an intrinsic part of the healthcare team in the long-term care facility. The Jeddah Institute for Speech and Hearing (JISH), a non-profit organization, was established in 1991, focusing on children in response to the absence in the Kingdom of qualified professionals and the great need for SLPs to help children with communication problems. Many parents had to take their children to the West for extended periods, and treatment was the only alternative available, simply out of reach financially for all but the wealthiest. The JISH is our flagship organization in Jeddah, the Kingdom’s second most important and international city. The JISH helps hundreds of children to realize their maximum communication potential, providing them with support with articulation, fluency, voice, receptive and expressive language delay, pragmatics, oral/aural rehabilitation, aphasia, etc. (JISH). The close, collaborative relationship between the KSA and the USA is reflected in the SLP community, particularly by the JISH, which provides ongoing intensive continuing education through courses, workshops, and internships at accredited university programs and clinics in the UK America. The JISH continues to grant scholarships to its staff for graduate and doctorate degrees in Speech-Language Pathology and Applied Behavior Analysis, helping to ensure that our speech-language pathologists are updated with the latest clinical techniques and stay abreast of research in the field. I laud the efforts of long-term care institutions such as NMC Chronic Care in Saudi Arabia to incorporate Psychologists and Social Workers into their patient care teams. This is a clear recognition that care includes and is based on communication. The most critical factor of all is the patients' communication with not only caregivers but also family. Especially concerning end-of-life care, for example, when “last words” are seen as having enduring importance, the patient must be able to speak or communicate in some fashion, and it is desirable to have a professional on hand to facilitate this. For this reason, I see the inclusion of an SLP professional on a long-term-care institution's team as integral and even foundational, helping patients or residents to communicate with caregivers and family. Without that capacity to make themselves understood and to convey their feelings, it isn't easy to know how to fully benefit from having both a social worker and a psychologist on a health care team. While a full recovery and the healing of independence lost is always the goal, especially for older adults who cannot adequately care for themselves, many, if not most, spend the balance of their lives in long-term care. I feel strongly called to advocate for the importance of the role of the SLP throughout the aging process, with even special attention paid to communication as a patient nears the end-of-life stages. I fully understand why our professional efforts in Saudi Arabia began focusing on children with speech and language issues, with their whole lives before them. But I hope to see the Med SLP working with people of all ages in need of their services, and I base this hope on concrete trends that we see developing today toward greater inclusivity and less age discrimination in the decision-making involved in who benefits from the services of a Med SLP, and who does not. Within my lifetime, I hope to see Med SLP services available to all who need them throughout the Middle East and the rest of the world. I see communication in all its forms, primarily verbal, to always be positive and not something that should be excluded from any program that considers the benefits for the patient from holistic care. I could not feel more strongly that the added value of the services of a Med SLP would certainly be worth the cost, reaping numerous dividends in terms of the quality of life of patients and residents as well as their caregivers whose lives are also improved by the general enhancement of the ability of the patient to communicate. The Med SLP that is most needed is well-trained. It is hoped that to the extent to which Med SLPs become increasingly valued by health care policymakers, the numbers of those members of this profession will increase over time, both as a result of growing numbers of Saudi students being trained abroad in this area as well as helping to an educated new generation in the field within the Kingdom. As populations age – and this is very pronounced in the KSA, SLPs are presented with a varied caseload that has become more acutely ill than in the past. Speech-language pathology services are increasingly becoming medically necessary to treat speech-language, swallowing, and cognitive-communication disorders common among patients with head injuries, Parkinson’s disease, stroke, autism, cerebral palsy, etc. Medical necessity is determined based on what is deemed to be essential and appropriate to the diagnosis and treatment of an illness, injury, or disease. Developmental disorders, conditions, disabilities, or delays are often accompanied by a natural progression in physical and psychological maturation from previous, lower, or embryonic stages to later, more complex, or adult stages, resulting in challenges that will exist throughout the individual's lifetime in question. Taking this into account and thinking about healthcare sustainably can make a compelling case that speech-language treatment services are as medically necessary for patients with developmental disorders that stay with them for a lifetime as they are for an adult who has suffered a stroke and subsequently lost speech and language function. It pleases me greatly that the Cambridge Medical and Rehabilitation Center (CMRC), one of the flagship long-term care organizations in the UAE, has opened a branch in Saudi Arabia. This organization does include Med SLPs on its team. Last year, in 2019, they opened a 60-bed facility between Dhahran and Al Khobar that helps patients requiring acute care and rehabilitation. They are already making a significant contribution to the Saudi healthcare system, serving as a model for future developments that include physical, occupational, and speech-language therapy, complete with multi-sensory room sessions (TVM Capital Health Care). This writer hopes that the role of the Med SLP will increasingly come to be seen as of vital importance to the team in long-term-care institutions of Saudi Arabia and the UAE, continuing to learn from the West and from each other as we march forward together in our advancement as SLP professionals. My ideal position would be serving as a Med SLP at the CRMC facility in the KSA, providing long-term care for patients with chronic illness or injury. I especially appreciate how the CMRC tailors its respect to the individual patient's treatment needs, encouraging the family to become involved in the patient’s treatment plans, including a rehabilitation plan for after the patient is discharged. A young woman from India, my education has increasingly come to embrace a focus on women in the workplace, women’s rights, liberation, economic development/participation, etc. As an undergraduate and master's degree student of economics, international business, and especially finance, the most vital aspect of my application is that I have teaching experience in my native India before I began building a new, married life in Canada, having served as an Assistant Professor of Finance and Management. All along the way, my particular focus has been on women, especially the critical role played by Finance in their economic empowerment in sustainable practices. I see women as the main economic motor of the future and the salvation of our planet; and my lifetime as dedicated to the cause of grass-roots economic development through the promotion of women-owned and led enterprises, working to enhance the empowerment of women, politically, legally, culturally, and most of all in the economic sphere. I spearheaded a committee in my college that organized seminars focusing on issues faced by women and girls, such as that many, if not most, girls drop out of school if they become pregnant due to social stigma. We organized collection drives periodically and provided local government schools with sanitary napkins and medicine for girls. Central to achieving greater economic independence for women is equal access to primary and higher education. I am heartened by the fact that the corporate world is beginning to realize the importance – and potential profitability, especially for the long term – of investing in the education of women and girls. Doing all that I can to advance this cause is central to my heart. As an undergraduate student, I surveyed local industries. Certain businesses were thriving because they implemented policies geared towards empowering women, especially by facilitating access to opportunities in higher education and providing equal opportunity for advancement within the company. I have understood better the big picture and how nothing can be adequately understood apart from the context in which it is located and how important it is that decisions be based on taking multiple perspectives into account. Teaching at my alma mater taught me that a teacher's job is not just to provide an articulate delivery but to learn to listen and respond constructively to feedback. Teaching polished my empathy improved my conflict resolution skills and helped me become a versatile professional. Developing my leadership skills helped me become highly effective at team building and management while still a student. After teaching, I appreciated how we must unlearn many things to find our own unique, creative voices. While teaching Management, I came to appreciate how social consciousness works more fully in tandem with strategic insight; realistic solutions can be found for complex challenges such as the recent scourge of COVID-19. As I draft this statement, India is closed due to this virus, and more than half a billion women and girls are turning their lives upside down.
