    In my second year of an Ob/Gyn residency in my native Ukraine, I met Maria, who was complaining of infertility and amenorrhea. After unsuccessful hormonal therapy, she was sent to our department for hysteroscopy. I entered the uterine cavity and felt like I had entered a haunted house for a moment . Multiple firm adhesions connected the uterus walls, all intertwined like a web. That was Asherman syndrome, which I had seen previously only in books. We almost gave up after several hysteroscopic resections, mechanical and hormonal treatment, and serial office hysteroscopies . Still, we decided to perform the last hysteroscopy by applying hyaluronic gel. Maria returned with a beautiful red-haired baby boy, Stefan, in her arms after a year . Now twenty-nine, I was born and raised in Odesa, Ukraine, a beautiful seaport with a diverse, international population; we have always been privileged as one of Ukraine’s top tourist destinations, which helped a great deal early on with my appreciation and celebration of diversity and multilingualism. My mother is Polish, and my father is Bulgarian and Romanian, contributing to my capacity to celebrate diversity in medicine. I graduated with my MD from Odessa National University in 2012. My husband and I first came to America and visited California as tourists. We fell in love with many things about California - people, nature, culture, food, and professional opportunities. So, we decided to move here 2.5 years ago and live near San Francisco. I hope to be selected for a residency position that excels at outreach, innovative research, and encouraging a team atmosphere with close contact between faculty and residents. I have been working on learning Spanish for some time now, and while I have a way to go, I am devoted to this cause because of the importance of Spanish for outreach to the underserved. I love to work with innovative technologies that help to improve health outcomes for patients. During my observerships in the United States, I have learned much about innovative technologies, medical ethics, and the US healthcare system. After giving my all to and completing a residency program, I also hope to go on to complete a fellowship in Minimally Invasive Gynecologic Surgery. After finishing medical school, I served at several hospitals in Odessa, one of which specialized in treating women with HIV facing high-risk pregnancies. I recognized the magnitude of effective communication with patients from diverse backgrounds, educating and empowering them, and providing treatment. I also spent a lot of time in trauma in Odessa, especially in the wake of political violence. I triaged the wounded and assisted in complex surgeries as part of specialist teams. In my free time, I volunteered in a women’s prison, performing general physical exams, pelvic exams, pap smears, etc., to ensure that the women received proper medical treatment. I decided to do this after discovering that these women usually receive care only in emergencies, with no preventive maintenance. Among my other preparations here in the USA, I am proud that I finished all steps of the USMLE in 2 years with above-average scores.     I hope to be selected for a Pathology residency program because of my great passion for research and teaching in my field. Nothing excites me as much as the possibility of participating in breakthroughs in Pathology that help result in sustainable life-saving changes that benefit millions. Serving as an integral part of a research team in Pathology is the noblest quest I can think of. I have felt this way for some time, which is why, after finishing medical school in India, where I was born and raised, I also completed advanced training in Pathology. I listened with rapt attention as he recounted the loss of my aunt, his daughter, to Retinoblastoma when she was only four years old. I cannot forget his sad face while he told us how much he loved her. His loss was due at least in part to my family’s humble economic circumstances. This touched me deeply, and I began to fully appreciate an illness's profound impact on the entire family. That night I made my decision to study medicine. My parents were proud of my decision. They have been the most noteworthy influence in my life. My mother put my education above her career, and my father worked day and night to pay for my tuition. Because of them, I am driven by positive emotions and an ethic of service to the community, especially its less fortunate members. I thrive on multiculturalism and diversity, especially since I was fortunate to have parents who lived in and visited numerous countries worldwide during my childhood for some of my formative years.     I trace my dedication to our battle against cancer to that day when I learned that my closest friend from medical school back home in India had been diagnosed with a mediastinal tumor and was undergoing chemotherapy. At first, I was in shock and disbelief and soon visited him in a hospital in New Delhi. I still remember his face's poised and serene look: courage and fortitude mixed with resignation. The whole time I was wondering what must be going through his mind. He told me how it started as an annoying cough, initially thought to be tuberculosis but eventually turned out to be poorly differentiated carcinoma. He underwent several cycles of chemotherapy, complete with the great suffering entailed, only to find out that the mass was chemo resistant. Nevertheless, he remained hopeful. One week after I had returned to the USA, I got a phone call and was told that he had passed away. It was hard to believe and accept that he would have to go at such a young age. This experience was the beginning of my total immersion in hematology/oncology, setting me on course for where I am now, hoping to be selected for your distinguished fellowship program. Furthermore, later that same year, my mother was diagnosed with breast cancer and underwent chemotherapy for more than a year. I took care of her, and it was painful to see her go through so much anxiety and suffer from the side effects of treatment. My mother is doing very well. I am deeply grateful for all the advances in oncology and to the people who contributed to those advances, as it provides me a chance to see my loved ones live longer and enjoy better health.     I trace my great passion for the practice of medicine to my keen desire as a young person to mature into a ‘good’ person, non-selfish, empathetic, compassionate, a good listener, and hungry to help ease pain and suffering. I am sometimes accused of being a perfectionist, but this is the price one pays for giving one’s all, one’s best, 24/7. I could not be more devoted to lifelong education and research in Internal and Critical Care Medicine . A team player to my innermost core, I am always open to constructive feedback from colleagues and mentors to constantly improve my capacity as a physician. I have completed my residency in internal medicine. I now feel most at home in the hospital at the center of the action. I am currently completing a fellowship in Neurocritical Care to benefit from total immersion into the foundations and challenges of Critical Care Medicine. During their stay at my home, I noticed that cardiovascular disease usually remained a long-term issue for patients who needed daily medicines and regular follow-ups. The other thing that saddened me was the long-term financial burden on patients with cardiovascular disease. Following my medical training in India, I decided to join the American healthcare system to learn advanced, evidence-based, research-oriented medicine. My hard work and dedication earned me a residency position at XXX. During my residency, I observed different outcomes among patients with cardiovascular disease. Patients taking preventive measures had better results in reduced hospital readmission, complications of the disease, and mortality than patients who were not following preventive measures. I had a patient who had three hospital admissions during the previous three months for CHF exacerbation. I counseled him regarding disease and the long-term benefits of preventive measures. We discussed steps to prevent hospital admission , including noting daily weight, fluid intake, salt intake, the importance of daily medication, etc. The patient said he felt much better and had no hospital admission three months later . This experience was rewarding and very satisfying. After observing improved outcomes among patients taking preventive measures, I was selected to join the Preventive Cardiology Fellowship at the XXXX Hospital. During my fellowship, I was fortunate to be accepted for the Applied Biostatistics course at Harvard University. The system gave me insight into various statistical methods and a better understanding of research outcomes. I started working on a research project focused on multiple dietary factors and cardiovascular outcomes. The American Heart Association accepted my research on coffee intake and heart failure risk among physicians . I am also excited about my forthcoming presentation on Healthy Lifestyle factors and risk of AAA at the “23rd World Congress on Heart Disease”.     I hope to be selected for an internship position at the Embassy of the Republic of Iraq in XXXX to gain the most valuable experience in project coordination and increase my engagement with people from my family's land of origin, Iraq. As I see it, my most important qualification is my passion for helping refugees from my native land, all Iraqis, and Arab speakers, not just Kurds.     I am a Hmong woman born to parents from Laos in a refugee camp in Thailand. Shortly after that, we were able to come to America, where we were given citizenship in 1994, making our home in California ever since. Growing up in the USA, my origins have increasingly informed my identity and sense of community and professional purpose – focusing on the Hmong communities of the USA. I hope to earn my master's degree in public health in preparation for a lifetime of service to my people, helping them understand and take advantage of healthcare resources here in the USA, our new homeland. I see San Francisco as the principal gateway to what is best in America, the most enlightened city with the most significant interest in humanitarian issues, and XXXX University as a flagship of progressive advancement in Public Health and the healthcare issues facing minority groups. In my parents’ country of origin, Laos, according to them, healthcare was non-existent for the ordinary Laotian. My parents had no formal education on how to care for their health. Instead, they sought consolation in their religious beliefs, which helped them feel more secure. Even today, the Hmong community in California remains isolated from the larger society and publicly available healthcare resources. My people, even in America, still suffer from a sub-standard knowledge of the causes, complications, and treatment of some of the most common health issues, such as high blood pressure, diabetes, and high cholesterol. I see the Hmong people in America as one of the most marginalized ethnic groups that make up our larger society. Therefore, as a member of this community, I hope to be selected for the MPH Program at XXXX University based on my experiences with my people and their challenges in public or community health. I lost my mother several years ago; she was only 59 years old and had been ill for many years. My mother will always be my main inspiration in death and life. She was diagnosed with high blood pressure, diabetes, and high cholesterol long ago, within a few years of our arrival in the USA, when I was just a child. However, my mother failed to adopt preventative measures and eventually suffered a stroke that would leave the right side of her body paralyzed; she just recently passed away from lymphoma. For the last two years of her life, she refused diagnostic tests because she feared medical intervention might make things worse: until her spleen became so big that it was causing complications. When she finally removed it, she would succumb one month after the surgery to lymphoma. She never had the chance to try chemotherapy or other treatment options. I cried. I felt overwhelmed and defeated. She was only 59 years old. Her death helped me to reaffirm my commitment to becoming an advocate for my underserved community. If she had trusted Western medicine more, she would have allowed herself to get treated earlier. If she had been more educated about the nature of the health challenges, she would have accepted that she needed medical and surgical intervention much sooner – or better still, to have adopted preventive measures while still young that would have added decades to her life. Still only twenty-four, I have worked hard to prepare myself for a career in Public Health, earning my bachelor's degree in health education with a minor in Child Development from CSU Chico. I have experience as a caseworker for the Asian community since I completed a 5-month internship as a Family Specialist (January-May 2017) with the Hmong Cultural Center of Butte County in Oroville, CA. The year before, 2016, I served as a Health Educator at XXXX Day Elementary School and a Healthy Community Volunteer with the Kids Farmers’ Market in Los Molinos, distributing fresh fruits and vegetables to students and helping them appreciate the importance of healthy eating.
